NOTICE OF INSUFFICIENCY. This is the second notice of insufficiency. The plaintiff has filed a motion for leave to proceed in forma pauperis ; in other words, he seeks to commence a federal lawsuit without pre-paying the filing fee. A federal law, 28 U.S.C. § 1915, permits him to do so if, among other things, he submits a financial affidavit showing that he is unable to pay, accompanied by a certified copy of his inmate trust account statement for the six month period preceding his complaint. On March 13, 2025, the Court issued a notice of insufficiency, stating that the plaintiff needed to complete and submit the correct application form, together with a certified copy of his inmate trust account statement, by April 2, 2025. (ECF No. 11.) On March 25, 2025, the plaintiff submitted another in forma pauperis application (ECF No. 15), but he did not follow the Court's instructions. He did not fill out the correct application form or submit a certified copy of his inmate trust account statement. He instead submitted a copy of his previous IFP application and a copy of his complaint, neither of which include the information the Court needs. The Court will give the plaintiff one more opportunity to submit the required materials. The Clerk of the Court is respectfully directed to send the plaintiff another blank copy of the correct application form at his address of record. The plaintiff is directed to fill out the form that the Clerk sends him , and not some other form, completely and accurately. The plaintiff must file the completed form, signed under penalty of perjury, with the Clerk of the Court along with a prisoner authorization and a certified copy of his inmate trust account statement covering at least the period from September 11, 2024 to March 11, 2025 . Alternatively, he may remit the full filing fee to the Clerk of the Court. If the plaintiff fails to either pay the fee or submit a complete application by April 16, 2025, his case may be dismissed. To assist inmates in demonstrating inability to pay, the District of Connecticut has prepared a standard-form in forma pauperis application. In this case, the plaintiff submitted an out-of-date version of the in forma pauperis application form. He also did not submit a certified copy of his inmate trust account statement for the last six months. His application is therefore insufficient. The Clerk of the Court is respectfully directed to send the plaintiff a blank copy of the correct application form at his address of record. The plaintiff is directed to fill out the form completely and accurately, and to file it with the Clerk of the Court along with a certified copy of his inmate trust account statement covering at least the period from September 11, 2024 to March 11, 2025.
